List ONLY volunteer activities that happened in December. Describe the activity in detail. Who were you with? What were you, specifically, tasked with doing? Was it an event? What was it for? Who is the activity benefiting?

Adult Contact: Make sure you get the name of an adult who can verify your hours AND their email address. If they do not have an email address, you can put a phone number, but email is preferred.

Hours: Round to the nearest 15-minute increment (15min, 30min, 45min, or 1hr).
